To fill out a 1099 Misc forms for pastors, I would suggest using a  form printing software.  W2 Mate  allows the user to fill in and print  1099 Misc forms and prints copies B,C, state 1 and 2 on blank paper as well as printing the 1099 Misc. Copy A and the 1096 on pre-printed forms which are required by the IRS.
